Projects for Assistance in Transition from Homeleness
Homelessness, in many cases, is due to individials or family members having mental illness and or substance abuse issues.
Many homeless individuals have become disenfranchised from the human services that are there to help them.
As a result, a large majority of this population becomes disengaged from their mental health treatment and do not receive the presciption medicines and counseling that help them maintain a healthy life style.
Daily Shower Program
Monday-Wednesday-Friday 8AM to 11AM
Assists with formulating an individual plan for PATH participants
Identifies resources for mental health treatment, substance abuse treatment, health care
Distributes blankets, personal care kits, list of Cochise County Resources, and HOPE
Assists with getting participants to medical appointments
Team is SOAR certified
Street and Desert Outreach visiting those experiencing homelessness "where they are at"
Accompanying participants to counceling and other medical appointments
Advocating for participants to become reconnected or help them to overcome systematic barriers
Helping participants to reunite ot reconnect with family
